To apply online you will need for yourself and each joint account owner:

A valid email address
Access to a printer
Social Security Number
Social security numbers of your designated primary/contingent beneficiaries
Valid drivers license OR other government issued non-expired ID
Must be Wisconsin or Florida Resident
Adobe Acrobat Reader (version 6 or higher). Download now.
One of the following Internet browsers:
- MS Internet Explorer (version 5 or higher). Download now.

Enrollment Process

Continue through online application. (If you have any questions, while completing this process, please contact one of our Representatives at 1-888-582-4440 or 920-459-4444).
You will be instructed to print, sign, date and mail your completed application.
Allow 2-3 business days for processing of your application.
Your account will be activated upon applicant verification, receipt of your deposit and a signed and dated application.
Notice of account activation will be sent to you at the e-mail address you will be required to provide.

Important Notice and Consent to Receive Electronic Documents
