Getting Started
We are glad you decided to open your Health Savings Account with Community Bank & Trust. If you have any questions before, during or after the online application process, please contact us at 1-888-582-4440 or 1-920-459-4444.
To apply online you will need:
- A valid email address
- Access to a printer
- Social Security Number
- Social security numbers of your designated primary/contingent beneficiaries
- Valid drivers license OR other government issued non-expired ID
- Must be Wisconsin Resident
- Adobe Acrobat Reader (version 6 or higher). Download now.
- One of the following Internet browsers:
Enrollment Process
- Continue through online application. (If you have any questions, while completing this process, please contact one of our Representative at 1-888-582-4440 or 920-459-4444).
- You will be instructed to print, sign, date and mail your completed application.
- Allow 2-3 business days for processing of your application.
- Your account will be activated upon applicant verification, receipt of your deposit and a signed and dated application.
Applicable Fees
- No set-up fee
- No monthly maintenance fee
- Free Debit Card
- $4.75 - Checks
- $15.00 - Annual maintenance fee (waived with a payroll direct deposit to checking account)
- $50.00 - Minimum deposit
HSA Eligibility
Prior to enrolling in an HSA, you must meet the HSA eligibility requirements. To determine if you are eligible to open an HSA, please answer the following questions.
Important Notice and Consent to Receive Electronic Documents
By clicking "I accept", you are agreeing to electronically receive documents in connection with your application. The electronic documents will include an application, disclosures and other forms that arise from the application process. "I accept" means that you have adequate computer hardware and software to have access to read and retain the documents. You agree to print the application documents and sign and date each document as required and return them to Community Bank & Trust. If you initially consented to receive the documents electronically, you may later request a paper form of the documents by contacting a Community Bank & Trust representative.

When you open an account, we will ask for your name, street address, date of birth, and other information that will allow us to identify you.
We may also ask to see your drivers license or other identifying documents.
